Get your plan ready for the meeting with the NDIS planner/LAC?

 

Our facilitators are independent and have no connection with any service provider or funding body, participants are free to discuss any situation in a safe and confidential environment. Our facilitators are experienced planners that will work with you and your unique set of circumstances to support you to describe your life right now and what it will take to get to a more desirable future.

We are inviting individuals with disability / mental health, their family members and their invited supporters to participate in the Pathway to a Good Life workshop.

Why is planning important?

We know that thinking about what you want in life and planning your goals is a positive step to:

♦  Assist you in your NDIS planning meeting
♦  Getting the support you want / need
♦  Creating a person centred plan with more choice and control built into it

This 2-day workshop can help you achieve that.

These workshops are interactive and full of examples of successful plans and approaches.

Workshops are facilitated by family members with experience in Person Centered Planning and Self-Managing supports, as well as experienced professionals from the disability/mental health industry.

The 2-days will cover these topics:

♦ How funding works under the NDIS
♦ What makes a good life?
♦ Circles of support and Micro-boards
♦ Describing how life is right now
♦ Developing a vision for the future
♦ Setting goals to achieve the vision
♦ What a typical week looks like now & in the future
♦ A budget to work towards
♦ Completing the NDIS paperwork
♦ Next steps towards Self-Managing the funding
